Open House 2021: Wellness Screening Questions

1) Are you experiencing the following symptoms?

• Cough
• Shortness of breath or difficulty breathing
• Fever of 100.4 F or higher
• Chills
• Muscle pain
• Sore throat
• New loss of taste or smell

2) Have you been in contact with someone known or presumed to have COVID-19 within the past 14 days?

If you answered “yes” to either of these questions, we request you not attend the Open House events in order to protect the other attendees.
